Event - I used to work in an industry which is very highly regulated by the FDA. Everything we did there was covered by extensive documentation. One day I noticed that Mike had missed documenting some instrument readings from the previous shift. I knew what a paperwork hassle this was going to create, along with management lectures to the whole department about the importance of documentation, so I called Mike at home and told him what he had done. The next morning, he came in and scribbled in some numbers and signed it off. He was caught and fired on the spot for false documentation.
Apology - I have felt guilty about this ever since it happened. Not because Mike got fired, he knew the risk and took it, but because I have always been afraid Mike thought I set him up and finked on him. Mike, it wasn't me! Lino saw the blank space and then that you had filled it in. You know Lino could never leave anything alone, he showed it to Rick who took it right to Larry. I would never have set anybody up that way.
Event - I was working for a lamp factory in the shipping department. I was just a kid and not ready for the job, but they hired me, so whatever. They got this massive order from Sears for about 10,000 lamps. I then proceeded to send them with the wrong shipping company and cost them about $18,000. I got fired, but I still felt bad about it.
Apology - The boss was a jerk. They treated me like cr*p. But I still was the one responsible for the mistake, so sorry about that.
